Root cause analysis in problem resolution

Don’t just patch bugs. You should understand why the problem occurred. You may use 5 Whys, Ishikawa, or similar methods - when this is beneficial. It is not neccessary to always employ these methods, but: Implement systematic root cause analysis. Identify systemic issues – not just symptoms.
